Owner Authorization for Tenant Water Service Form

Please note that this form does not transfer service, it only authorizes your tenant to initiate service with us. Your tenant must submit an application, pay the application fee, and provide a deposit if required. A new account will be created in their name only after all these steps are completed. Until then, the active water account will remain in your name. 


FORM MUST BE FILLED OUT AND SUBMITTED BY OWNER OR PROPERTY MANAGER ONLY - NO EXCEPTIONS

I am the owner/property manager of the service address shown above. I understand per Joshua Basin Water District Rules and Regulations that the property owner is ultimately responsible for payment of all applicable water charges. I further understand that Joshua Basin Water District will not activate any new accounts on a property with a delinquent balance, and all account balances must be paid in full prior to establishing a new service. If all outstanding charges are not paid, the property owner must open and maintain the account(s). The property manager/owner will be notified if the tenant account becomes subject to lock off for non-payment. The property manager/owner will also receive a copy of the tenant’s final bill for reference. I authorize Joshua Basin Water District to furnish water in the tenant’s name. I hereby guarantee payment of all charges for any accounts listed at the above address. I understand and agree that this authorization is valid until revoked by the property owner or terminated by Joshua Basin Water District.
